Giving Form

Giving to Allegheny General Hospital is easy.  Simply print the AGH Giving Form, complete the information and mail it along with your check or credit card information to the address below.  If paying by credit card you may opt to fax the completed form directly to the Office of Fund Development.  Checks should be made payalbe to Allegheny General Hospital.  If you have any questions feel free to contact us by phone or email.
