The registration fee includes the course materials, continental breakfast and break refreshments.

Other Health Care Professionals:  $10
General Public and Nonmedical Professionals:  No Charge


April 30, 2013 (Registrations will be taken after the deadline date on a space-available basis.)

Refund Policy

A refund of the registration fee, less a $10 processing fee, will be made when a written request or a telephone request followed by a written confirmation is received prior to April 30, 2013. After this date, refunds cannot be made. Fees will not be refunded for failure to attend.
