Pennsylvania Orders for Life Sustaining Treatment (POLST)

Honoring Patient Treatment Wishes at the End of Life

This course, designed for home nurses, provides general instruction on the POLST form. Through a self-guided PowerPoint presentation and a video, participants gain an understanding of the form’s rationale and the context for its use. These topics will be presented in contrast to Advanced Directives to distinguish between the two forms. In addition to learning the specific details and requirements of the POLST form, participants will also learn how to effectively initiate and hold end of life discussions with patients.

  • Understand appropriate framing for POLST conversations
