Application and Enrollment

Instructions for Applicants

1. Complete the application:
   
   A. Download and complete the Application for Admission.  

   B.  Read the Disclosure Statements regarding:

1. Retention rate and cohort completion rate
2. Result of achievement on standardized tests
3. Number of graduates
4. Pass rates on NCLEX
5. Employment rate for graduates
6. Felonious acts

C.  Sign and date the application.

D.  Enclose the non-refundable fee of $50.00 in the form of a check or money order made payable to: The Western Pennsylvania Hospital. This fee is for application to both the West Penn Hospital School of Nursing and Clarion University.

E.  Send the signed application and fee to:
     Admissions Office
     The Western Pennsylvania Hospital School of Nursing

     Pittsburgh, Pennsylvania 15224

2. Request that the following materials be sent to the School of Nursing:
   
   A.  Official high school transcript or G.E.D. scores (if applicable).    Request the school to include:
          1. cumulative GPA and
          2. graduation class rank.
   B.  Official transcripts from all post-secondary educational institutions.
   C.  SAT scores (if not printed on high school transcript).
    
3. Download and print out at least four (4) copies of the Reference Form.
   
   Four references are required from individuals such as:  teachers, counselors, employers, nurses or clergy.  References should not be from relatives, friends, or acquaintances.  References should be returned to the School as soon as possible to facilitate the application process.

Enrollment Information

Enrollment is contingent upon successfully meeting academic, personal, and health requirements.

To confirm enrollment, a $95 non-refundable fee by check or money order must be sent to West Penn School of Nursing by the date specified in correspondence to accepted applicants.
