ACLS provider status must be current to register for the renewal. If your last ACLS class was not through the AGH Training Center, please fax a copy of your card, front and back, to 412.359.6914.
A current provider card is required for participation in any renewal program.
Employer information MUST be included in the comment section of the on line registration. This is a "must enter" field and the form will not be submitted if left blank.
