Hospice in Long Term Care Facility



When care cannot be managed at home a loved one may require a nursing home or assisted living facility.  Your choice of nursing facility care came out of love, respect, and concern for their well being.  No matter where “home” is, we feel at Forbes Hospice and Palliative Care that everyone faced with a serious, progressive, life limiting illnesses have the right to hospice care.  Hospice care at a facility is an additional layer of support for your loved one focusing on pain and symptom management as well as spiritual and emotional support. 

Forbes Hospice and Palliative Care utilizes an interdisciplinary team approach to care.  Our care team consists of a palliative care certified physician, registered nurse, social worker, certified nursing aide, spiritual care counselor, bereavement coordinator, and trained volunteers all working with the health care team at the facility to provide excellence in care.  We do not forget that the resident and the family members are part of the team too.  Your wishes and concerns need to be heard and understood. 

Medicare and most commercial insurances have a hospice benefit which covers the cost of this care.  The only exclusion is people receiving insurance coverage for skilled care as Medicare and insurances will not pay for both skilled care and hospice.  But residents faced with life limiting illnesses can benefit from hospice care.
