Application for Medical Student Rotations
Click on the link below for the application form for students attending an LCME- or AOA-approved medical school to complete a rotation at West Penn Allegheny Health System. Applications must be received six weeks prior to the requested rotation:

The following information is also required for application:
- Immunization Record
- Letter of Good Standing from the Medical School
- Proof of Liability Insurance
- Proof of Personal Health Insurance
- Child Abuse, Criminal History and Fingerprint Reports

When requesting more than one rotation, an individual application must be completed for each requested elective but if several rotation requests are sent at the same time, only one set of the additional information is needed.
