Palliative Care Program
Our Palliative Care program provides for the physical, emotional, and spiritual needs of patients, families and loved ones dealing with serious, progressive illness such as:
- Cancer
- Chronic lung diseases
- Heart disease
- Liver disease
- Kidney failure
- Neurological diseases
- Other end-stage illnesses
We provide palliative care on an inpatient and outpatient basis, helping families coordinate services for their loved ones when they are hospitalized, require home care, hospice services, or transfer to inpatient facilities. The palliative medicine team assists patients who are recently diagnosed with a serious illness and offers assistance throughout various phases of the disease including symptom management, goals of care and coordination of services.
Our services include:
- Pain and symptom management
- Counseling in treatment choices and decisions
- Emotional and spiritual support
- Patient and family counseling
- Bereavement support and referral services
- Planning for post-hospital care
