Your Care Team

Medical Director – Certified in Hospice and Palliative Medicine, our Medical Director is available for home and inpatient unit visits and communicates with your physician.

Registered Nurses – Our nurses are experienced and certified in hospice care. Based on a physician care plan, they provide care and comfort in many ways. They evaluate and treat pain and other symptoms such as nausea, shortness of breath, anxiety and restlessness. Our nurses will educate, inform and provide a loving presence in your home.

Certified Nursing Assistant – Charged with the most personal of services, our certified aides provide personal care, bathing, dressing and feeding assistance. They are a great resource to caregivers on the tricks-of-the-trade to care for a loved one.

Social Workers – The effects of illness go beyond the physical and often times family and emotional issues are present. Social Workers are trained counselors who can help with emotional support, counseling, financial assistance, care planning, education and community resources. Hospice care is for everyone involved.

Hospice Chaplains – The end of life often brings a desire for greater spiritual awareness. Our chaplains, who are interdenominational in their approach, are available to visit and communicate with your pastor, rabbi or clergy, if desired. Our spiritual counselors are ready to comfort and allow you to share your concerns.

Volunteers – Our volunteers enhance the program through their tireless commitment to the patients and families we serve. We regard volunteers as essential in carrying out our mission - offering an extensive volunteer training twice a year. Volunteers are involved in every aspect of the hospice program, including home visiting, inpatient care, 11th hour program, spiritual care, nursing home visits, administrative support, bereavement support and fundraising.

Bereavement Counselor – The care of your loved one is only part of hospice care. The care of you, especially after the death of your loved one, is paramount. Grief support through individual counseling, support groups, phone calls and mailings are available. We also have memorial services honoring our hospice family. These bereavement support services are available to family members and friends.
