After scheduling an appointment, please print and complete the patient history form if you haven't already received a packet in the mail. There will also be a registration sheet and consent to treatment forms to fill out once you arrive at the office.

Call our scheduling offices between the hours of 8 a.m. and 5 p.m. (Eastern Standard Time) Monday through Friday to refer a patient.
