Giving to Forbes Regional Campus
Tree of Life
Honor or memorialize a loved one, physician, staff member or friend with an engraved brass leaf, small bronze stone or large bronze stone on or at the tree base of Forbes Regional Hospital’s or Forbes Hospice Tree of Life. The tree contains personalized inscriptions on the leaves and stones surrounding its base.
Leaves and stones that make up the tree may be purchased through donations at specified levels.
Donation Levels and Inscription Guidelines
Brass Leaf (4” x 11/2”)
Donation of $400, up to 65 Spaces
Small Bronze Stone at Base of Tree (4” x 3”)
Donation of $1,500, up to 100 Spaces
Large Bronze Stone at Base of Tree (5” x 4”)
Donation of $2000, up to 100 Spaces
Inscriptions may be specified at time of donation.
Baby Steps Keepsake Tile Program for New Parents and Grandparents

Forbes Regional Hospital offers you the opportunity to commemorate the birth of the newest member of your family, with a place on our baby steps wall. Proceeds from this program benefit the woman and infants care center at Forbes Regional Hospital.
Your donation includes 4” x 4” ceramic tile with the footprint, name, birth weight, and birth date of your baby, which is displayed in our Women and Infants Care Center. You also receive a duplicate keepsake tile, as a permanent reminder of the special day that your little one was born. Additional quantities of the same tile can be yours for an additional donation of $45 each.

Patient and Family Support Services Fund
Donations to The Patient and Family Support Services Fund cover services that are not covered by any other programs, and include meals, transportation, temporary housing and medications.
Nursing Education Fund
Contributions to the Nursing Education Fund, enable us to provide staff with advanced education, allowing us to continue our mission of providing quality patient care, advancing medicine through medical research and educating the next generation of health professionals.
Hospice Respite Care Fund
The Hospice Respite Care Fund pays for in home respite care for family caregivers caring for homebound patients in Forbes Hospice. This fund allows us to provide, free of charge, limited respite hours at home primarily staffed by certified nursing aides. The home respite program is above and beyond what is required by a Medicare-certified program but it is an added benefit of choosing Forbes Hospice and Palliative Care.
