Giving Form
Giving to the Temple University School of Medicine at West Penn Allegheny Health System (TUSM at WPAHS) is easy. Simply print the TUSM at WPAHS Giving Form, complete the information and mail it along with your check or credit card information to the address below. If paying by credit card you may opt to fax the completed form directly to the Office of Fund Development. Checks should be made payable to the TUSM at WPAHS.
